　　China has the world’s biggest diabetes epidemic(流行病;迅速传播), and it continues to get worse, according to the latest study of the disease’s devastating (惊人的;可怕的)effects on the world’s most populous (人口密集的)country, which has risen from poverty to become an economic superpower(经济强国) in 30 years. The number of people with diabetes in China is estimated to be 1 in 10 out of Chinese adult. Diabetes has become a major public health crisis in China, with an annual projected (预算的;估计的)cost of 360 billion RMB (almost 60 billion USD) by 2030.

　　Assessment data

　　Mrs. Taylor, age 53 years, is admitted to the hospital for a hysterectomy. She has had Type I (insulin dependent) diabetes since the age of 20. Mrs. Taylor has recently started blood glucose monitoring at home and has achieved fairly good control of her diabetes. Her blood sugars usually are withinnormal limit; her blood lipids remain at acceptable levels. She is about 10 pounds’ overweight, and in general she has a sense of health and well-being.

　　The morning of her surgery an intravenous infusion of 5% dextrose was started and short-acting insulin was added to the infusion. On her return from surgery the physician ordered the intravenous infusion continued for 24 hours, blood glucose determinations q4h, and subcutaneousadministrations of regular insulin based on the blood glucose.The following day intravenous feedings were discontinued and blood sugar determinations were ordered twice a day. Mrs. Taylor’s usual dosage of insulin wasresumed, with additional units of regular insulin as indicated by blood glucose levels.

　　1. Monitor closely for signs of severe hypoglycemia and hyperglycemia.

　　2. Measure intake and output.

　　3. Monitor IV infusion at least q 1 h to assure proper rate of flow.

　　4. Check lab results for blood glucose at 8:00, 12:00, and 4:00. Administer insulin promptly.

　　5. Be especially alert for insulin reaction as she recovers from the stress of surgery.

　　6. Scrupulous wound care, aseptic technique for dressing changes.

　　7. Monitor blood sugar levels as guide.

　　8. Note and document status of wound and color, appearance, amount, and odor of drainage.

　　9. Inspect venipuncture sites once each shift for signs of infection.
